Fenotipos y fenocopias bipolares y su variación en el ciclo de vida

Resumen

Introducción: Son escasos los estudios que abordan el trastorno bipolar (TB) desde una perspectiva de psicopatología del desarrollo y, por lo tanto, se sabe poco sobre cómo se presentan los síntomas de esta enfermedad a lo largo del tiempo, en las diferentes etapas del desarrollo humano. Metodología: En esta revisión narrativa describiremos las presentaciones inusuales de la tuberculosis (“disfraces clínicos” o fenocopias) a lo largo del ciclo de vida y sus implicaciones para el diagnóstico diferencial y el pronóstico. Resultados: Se describen los principales disfraces psiquiátricos vinculados a la tuberculosis a lo largo del ciclo vital. Las dificultades semiológicas y, en consecuencia, los errores diagnósticos asociados a la tuberculosis, que a su vez se reflejan en el buen funcionamiento y aumento de los costos de la salud pública. Conclusiones: Otros trastornos mentales y enfermedades neurológicas pueden reconocerse erróneamente como TB, ya que el diagnóstico aún está respaldado por datos clínicos. La investigación traslacional puede permitir en el futuro la identificación de biomarcadores y mejorar el diagnóstico diferencial. También es importante considerar las etapas clínicas de la enfermedad en muestras bien caracterizadas de jóvenes con alto riesgo durante el diagnóstico.
