Bell's mania, also called acute delirious mania and other nomenclatures, is a severe clinical case, whose etiology and pathophysiology are not widely known. It is characterized by the sudden and simultaneous manifestation of the symptoms of mania and delirium. Currently, there are no consensual diagnostic criteria, and therapeutic guidelines for its proper management, moreover there are rare epidemiological data and case reports in scientific literature. We report the case of a 31-year-old female patient with acute delirious mania who received psychopharmacological and multiprofessional treatment during two hospitalizations in a psychiatric hospital. Although there are controversies about the treatment of this form of mental illness, there was stabilization and remission of symptoms once the benzodiazepine was introduced, with favorable longitudinal evolution through the prescription of valproic acid associated with clozapine.
