The era of antidepressants

Objective: To detect, through the analysis of the historical evolution of antidepressants (ADs) and their putative mechanisms of action, if there is a scientific basis that guides their clinical use. Method: analysis of relevant clinical studies and systematic reviews published over the last sixty years on the mechanisms of action and clinical use of ADs, their relationship with diagnostic classification systems in psychiatry, the role of regulatory agencies and the evolution of the system current patents. Results: it is not possible to justify with the available clinical-scientific data the clinical choice of this or that group of ADs. Conclusion: Open, double-blind, randomized clinical studies do not lend themselves to choosing the clinical use of a given AD. As “meta-analyses” are based on these studies, their results are conflicting. The choice of an AD continues to be based on the psychiatrist's experience.
