Resumen

Objetivo: Detectar, a través del análisis de la evolución histórica de los antidepresivos (AD) y sus supuestos mecanismos de acción, si existe una base científica que oriente su uso clínico. Método: análisis de estudios clínicos y revisiones sistemáticas relevantes publicados durante los últimos sesenta años sobre los mecanismos de acción y uso clínico de los DA, su relación con los sistemas de clasificación diagnóstica en psiquiatría, con el papel de las agencias reguladoras y con la evolución del sistema actual patentes Resultados: no es posible justificar con los datos clínico-científicos disponibles la elección clínica de tal o cual grupo de EA. Conclusión: Los estudios clínicos aleatorizados, abiertos, doble ciego, no se prestan a la elección del uso clínico de un determinado DA. Como los “metanálisis” se basan en estos estudios, sus resultados son contradictorios. La elección de un AD sigue basándose en la experiencia del psiquiatra.
