Tratamiento del trastorno de estrés postraumático complejo en niños y adolescentes según CIE-11: revisión de la literatura

Introducción: Más del 60% de los jóvenes reportaron haber vivido al menos una experiencia traumática durante la infancia. El Trastorno de Estrés Postraumático Complejo (TEPTC) es un nuevo diagnóstico de la CIE-11, con pocos estudios publicados sobre su tratamiento, especialmente en niños y adolescentes. Objetivo: Revisar la literatura disponible sobre el tratamiento del Trastorno de Estrés Postraumático Complejo en la Infancia y la Adolescencia (TEPTC-IA) y definir las mejores estrategias para su tratamiento. Método: Através de una revisión narrativa, incluimos artículos que abordaron el tratamiento del TEPTC-IA. Se realizó el levantamiento de dos artículos publicados en la literatura en las bases de datos: 1) BVS – Biblioteca Virtual en Salud, y 2) PubMed, con las palabras, sus variantes y siglas para Trastorno de Estrés Postraumático Complejo en Niños y Adolescentes y “Complex Post-Traumatic Stress Disorder in children and adolescents”. Resultado: Se incluyeron 5 artículos publicados sobre el tratamiento del TEPTC-IA, con énfasis principalmente en la Terapia Cognitivo Conductual Centrada en el Trauma (TCC-CT). Conclusión: El presente estudio encontró que la TCC-CT es la terapia más estudiada y efectiva para el TEPTC-IA, por lo tanto, los síntomas del TEPTC, como la desregulación afectiva, aún causan una angustia relevante después del tratamiento. Los estudios analizados sugieren la necesidad de mejorar el tratamiento, como una mayor individualización de la terapia, con énfasis en los aspectos más deficientes de cada paciente, como una TCC-CT más prolongada en la fase de tratamiento de la desregulación afectiva y el funcionamiento relacional. Un estudio de la Terapia de Procesamiento Cognitivo Adaptada al Desarrollo (TPCAD) demostró eficacia para TEPTC-IA.
