Management of a patient with schizophrenia associated with Crohn's disease: case report

Introduction: Schizophrenia is a psychiatric illness characterized by cognitive, affective and psychotic disorders, being one of the most serious mental disorders with social and psychological impact. Method: Case report. Associação Evangélica Beneficiente de Londrina, AEBEL. Approval Research Ethics Committee (REC) n. 7.122.704. Case presentation: This article describes the case of a patient diagnosed with paranoid schizophrenic disorder and suffering from serious medical comorbidities. The patient presented psychotic symptoms with auditory hallucinations, persecutory delusions and disorganized behavior when he sought the city's psychiatric hospital. Despite regular monitoring at the Psychosocial Care Center (CAPS) and the use of conventional antipsychotic medications, there was no significant improvement in symptoms. During hospitalization, compromised gastrointestinal absorption was diagnosed due to the progression of Crohn's disease, which influenced the choice of drug therapy. Multidisciplinary treatment was crucial for a comprehensive approach to the patient. Result: The introduction of injectable depot medication, such as Zuclopenthixol (Clopixol Depot®), demonstrated a substantial improvement in psychotic symptoms, indicating the importance of concomitant medical conditions in the management of schizophrenia. Conclusion: The clinical management of patients with schizophrenia and severe medical comorbidities requires an integrated and individualized approach. Consideration of concomitant medical conditions, along with assessment of psychiatric symptoms, is essential for therapeutic success.
