Resumen

Aunque el estado maníaco es la condición definitoria para el diagnóstico del trastorno bipolar, el estado depresivo es la condición sintomática más frecuente en la vida del paciente y es responsable de la mayor parte de la incapacitación y sufrimiento generado por las personas con trastorno bipolar (Goodwin & Jamison, 2007 ). En un estudio de seguimiento de 10 años, los pacientes con trastorno bipolar I estaban deprimidos el 31% del tiempo, más que en manía (9%) o estados mixtos (9%) (Judd et al., 2003a). Los pacientes con trastorno bipolar II padecían depresión el 50,3% del tiempo, mucho más que hipomanía (1,3%) (Judd et al., 2003b). Alrededor del 50% de los pacientes con trastorno bipolar tienen un episodio depresivo como primera manifestación de la enfermedad, y los episodios depresivos pueden ser crónicos en el 20% de los pacientes y ser más refractarios al tratamiento que los episodios maníacos o hipomaníacos (Yatham et al., 2005). . La depresión bipolar, ya sea el episodio depresivo o los síntomas subsindrómicos, también contribuye a gran parte de la pérdida de funcionalidad y calidad de vida de los pacientes con trastorno bipolar (Goodwin & Jamison, 2007).
