Psychiatric manifestations in rheumatology

The present literature review investigated symptoms related to Rheumatoid Arthritis and Systemic Lupus Erythematosus. We believe that a new paradigma must be established in psychiatry, considering that the relation between rheumatological diseases and psychiatric symptoms should be highlighted, helping to refine diagnosis and therapeutic proposals in mental health. Thus, we seek to trace evidence of psychiatric conditions prototypes that are more characteristically associated with the aforementioned rheumatological conditions. Symptoms related to mood disorders, mainly depression, were strongly associated with the higher incidence and morbidity in Rheumatoid Arthritis; besides, cognitive deficits, especially on moderate levels, are the most prevalent neuropsychiatric symptoms in Systemic Lupus Erythematosus. Pharmacological treatment of the underlying rheumatological disease is essential to improve the associated psychiatric condition, whether by direct or indirect influence of such condition. As for Lupus, current therapeutic strategies are largely empirical, carried out through known immunopathogenic mechanisms and results observed from treatment of severe manifestations in other organs. Regarding Rheumatoid Arthritis, the specific pharmacological treatment of the psychiatric condition is essential according to the individual demand of each patient, and intersubjectivity and the psychosomatic aspects should always be taken into consideration.
